Please see the main office of PHS for parking pass information
- Completed Forms: Please plan to provide proof of current insurance (we don't need a copy, just need to see proof) and valid drivers license. Return completed forms to the main office at PHS with signatures from both parent/guardian AND student.
- Parking Permits will be issued on a first come first served basis.
